Forms and Certificates
The Petroleum and Gas (Production and Safety) Act 2004 provides the framework for ensuring the safe exploration, production, transportation and utilisation of petroleum and gas in Queensland.
The Petroleum and Gas (Production and Safety) Act 2004 commenced on 31/12/2004. Among other things, this Act replaces the Gas (Residual Provisions) Act 1965. The Petroleum Act 1923 has also been amended.
The Petroleum and Gas (Production and Safety) Regulation 2004 also commenced on 31/12/04 and provides the more detailed regulatory requirements for safety matters.
Under the legislation and regulation, a number of activities require forms to be submitted to the department or certificates to be issued to owners and operators of gas systems. The relevant safety forms or certificates must be in a form approved by the Chief Inspector.
Forms
The following forms are either approved forms under the Act or are forms prepared by the department for use in administering the Act and Regulation. To ensure validity, always access required forms via this site or from offices of Queensland Mines and Energy, Department of Employment, Economic Development and Innovation. Note: these pdf forms now can be saved and will allow electronic entry in each field. The saved pdf can then be printed and lodged.
